OCTO partnered closely with Plans4Care’s clinical experts to define the product strategy and roadmap for bringing their proven care methodology to a digital platform. Together, we structured a scalable product vision that prioritized personalization, accessibility, and continuous caregiver support.
Working iteratively across strategy, design, and development, we transformed decades of evidence-based clinical practices into an intuitive digital experience. At the core of the platform is a personalization engine that delivers tailored caregiving strategies based on a person with dementia’s ability to manage daily activities. The app provides guidance across more than 100 identified care challenges and over 2,000 clinically validated strategies—helping caregivers respond confidently to evolving situations at home.
